Before Your First Appointment
To save time during your first appointment with Dr. Scott Sigman, we request that patients download and fill out the form below and then bring it with them to the office. This will provide Dr. Sigman and his staff with your background information and allow them to get a head start on any necessary paperwork, helping your first appointment run as smoothly as possible.
We see patients in our office Monday through Friday, 8am-5pm. To schedule an appointment, please call or fill out the online appointment request form and one of our staff members will be happy to help you. We do our best to accommodate the busy schedules of our patients. We request that patients provide us with 24 hour notice if they need to cancel an appointment.
Patients with an HMO insurance plan will be required to get a referral from their primary care doctor before receiving treatment from Dr. Sigman.
Patients of Dr. Sigman can request routine prescription refills by calling the office or notifying their pharmacist. For same-day refills, please call the office before noon if possible. Any calls received after 2:30pm will be fulfilled the next business day.
Please have the following information available so that we can best complete your prescription refill request:
- The patient’s name, address, date of birth, cell phone number and email address
- Name of medication
- Any allergies
- The pharmacy name and phone number
Routine prescription refills cannot be made during evenings, weekends or holidays, so please plan accordingly and call us during a weekday before your medication has run out. We cannot call in prescriptions for pain medication narcotics over the weekend or after 5pm during the week – patients will need to make an appointment to see the doctor. In urgent cases, patients should visit the emergency room.
Payment & Insurance
Dr. Scott Sigman is a member of most Preferred Provider Organizations (PPO) insurance plans, and also accepts patients with Medicare and Worker’s Compensation.
We strive to provide all of our patients with affordable medical care. In order to do so, we request that payment arrangements be made at the time of services rendered. We accept cash and most major credit cards for the convenience of our patients. We will also bill your insurance company and assist in handling claims in every way we can, but responsibility for payment ultimately falls to the patient.
Our main focus is to provide our patients with the appropriate medical care for their individual needs. Our billing office will work with you regarding any special needs or concerns you may have.
